An incident is something which has caused or may cause unnecessary harm. Reporting incidents and assessing risk is a vital part of the Trust’s processes for improving safety and quality.

Reporting incidents help us to:

  • Learn from incidents and near misses as a Trust - whether we need to change our processes to improve patient safety
  • Identify where we need to focus resources, such as training and finances
  • Review individual service user incident histories so we can review care plans and risk assessments
  • Measure our performance against our aims to reduce harm from incidents
  • Report to meet our statutory and legal requirements to record incidents
  • Understand our gaps and vulnerabilities – so we can formulate a risk register
  • Report our patient safety incidents and near misses to the National Reporting and Learning System (NRLS) so that information can be reviewed nationally for trends or problems, for example with equipment failure

Ulysses Safeguard is our online reporting system for reporting incidents. Whilst on placement, you can observe and learn about the incident reporting process, however you should not be completing a Ulysses report.

If you are directly involved in any incident that requires reporting, please notify your university as soon as possible.

As student we encourage you to raise any concerns you have to your Practice Supervisor or Assessor in the first instance. If you feel unable to do this then please contact the Learning Environment Team directly be emailing:

  • Be prepared and organised for your placement. Ensure you have access to RiO, collate evidence of learning throughout your placement (feedback, reflections, research) and ensure your interviews happen on time

Attendance

  • Expect to work allocated/shifts and duties across a 24 hour time period to provide all aspects of care (predominately nursing placements)
  • Please arrive on time, allowing time prior to your shift/start time to settle yourself in
  • Please do not rearrange shifts without consent of your Practice Assessor or student link
  • Please inform your placement area if you are running late, sick or absent in good time
  • Please also let your university know if you are sick or absent

Appearance

  • Adhere to 'uniform policy'  (found on the Policies page here) at all times
  • Maintain a professional image
  • Bare below the elbow in line with 'Infection, Prevention and Control Policy' (found on the Policies page here).
  • Ensure your student ID is visible

If you need any reasonable adjustments, please could you share these with the placement area, supervisor/assessor at the first interview or earlier.

As students you play a key role in telling us how we are doing. So please complete your feedback at the end of your placement good or not so good to help us improve our placement experience for students. Your teams will receive your feedback!

If you have a good Assessor or Supervisor please feel free to mention them by name or email them personally as this can be useful for NMC revalidation.

National Education Survey

The National Education Survey (NETS) gathers opinion from students about their experiences in clinical placements:

• what worked well

• what could be improved

These results help us all, placement providers, universities and Health Education England to maintain the quality of the learning environment for future student. So please take the time to complete this after your placement. National Education Survey
